How the note is written
Every sentence comes from the numbers you typed: independent accuracy, accuracy with cues, which cue level you used, and how the result compares with the mastery criterion. The Assessment and Plan lines follow the usual school-SLP conventions — continue and raise complexity at criterion, fade cues when emerging, reduce the set when below. It adds no observation you did not make.

- Is the SOAP note generator free?
- Yes. The generator needs no account and no login. Type your trial tallies, pick SOAP, BIRP or DAP, and copy the note. The optional AI rewrite is free too, limited to 5 drafts an hour per connection.
- Is anything I type stored?
- The rule-based note is written in your browser and never leaves it. If you press Improve with AI, only the note text is sent to write the draft, and nothing is saved. Leave student names out; the tool never asks for one.
- What is the difference between SOAP, BIRP and DAP notes?
- SOAP splits a session into Subjective, Objective, Assessment and Plan. BIRP records Behavior, Intervention, Response and Plan. DAP folds the subjective and objective parts into one Data section, then Assessment and Plan. All three can carry the same trial data.
- Where do the accuracy numbers come from?
- From the counts you enter for each target: correct, correct with cues, and incorrect. The note reports independent accuracy, accuracy with cues and the cue level you used. It adds no observation you did not make.
- Can I use the note for IEP progress or Medicaid documentation?
- It gives you a draft with the session's data in it. You remain responsible for the final note: check it against your district's template, your state's Medicaid documentation rules and the student's IEP goals before you sign it.
- Which goal areas does it support?
- Articulation, language, fluency, voice, pragmatics, phonological awareness and AAC. Pick the area and you can add a goal from the free goal bank to the note in one step.
